LEAVE DONATION PROGRAM
STATE EMPLOYEES
UUP, MC, CSEA, PEF, NYSCOPBA, and PBANYS Employees
The intent of the Leave Donation Program is to provide a means to assist employees who, because of a long term personal illness, have exhausted their leave benefits and would otherwise be subject to a severe loss of income during a continuing absence from work. The Leave Donation Program enables employees to donate Vacation leave credits to co-workers in order to keep them in pay status during a period of personal illness.

ELIGIBILITY
UUP or M/C Employees
Eligible at the time of hire. Employees that are eligible for both Presidential Sick Leave and Leave Donations must first apply (or waive in writing)for Presidential Sick Leave to an HR Leave Administrator.
CSEA, NYSCOPBA, PEF, and PBANYS Employees
Eligible at the time of hire. Employees that are eligible for both Sick Leave at Half Pay and Leave Donations must first apply for Sick Leave at Half Pay or waive the use of Sick Leave at Half Pay in writing to an HR Leave Administrator. An employee who opts to use Leave Donations prior to Sick Leave at Half Pay is permitted to again use Leave Donations following the exhaustion of Sick Leave at Half Pay.

Provide satisfactory medical documentation and fax or email copy of the Leave Donation Recipient Form to an HR Leave Administratorat hrs_timeatt@stonybrook.edu. Donor Forms must be completed by the employee donating time and can be faxed or emailed to hrs_timeatt@stonybrook.edu.
