Students and alumni applying for professional licensures, certification, or credentialing may be required to provide official verification of enrollment or degree completion.

Stony Brook University supports these requests through transcript services and certification forms, depending on the requirements of the licensing agency.

A professional license authorizes an individual to practice within a regulated field and to use a designated professional title.

In New York State, licensure applications are reviewed by the Office of the Professions.

Completion of Licensing Forms

Some licensing boards require specific forms to be completed by the University.

Students must complete all required student sections before submission

• Forms should be submitted to your professional program as the first step for processing
• The professional program will work with the Office of the Registrar in completing the necessary sections.
• The Office of the Registrar will certify the form and mail to the NYS Office of Professions
• Processing times may vary depending on the complexity of the request
• Additional documentation or verification may be required based on the licensing agency’s criteria.
