ELIGIBILITY

Eligibility

You are eligible to enroll in the health insurance plan if ALL of the following conditions are met:

  1. You are appointed as a Graduate Assistant or Teaching Assistant.
  2. You work at least a one-half assistantship.
  3. You are employed at an annualized stipend of $4,293.
  4. You are in an active pay status.
DEPENDENT ELIGIBILITY

You may enroll the following dependents in your health insurance plan:

  1. Your legal spouse.
  2. Your unmarried children under age 26.
  3. Your Domestic Partner (contact HR Benefits at (631) 632-6180 for details on required qualifying criteria).
EFFECTIVE  BEGIN/END DATES

Coverage begins on the first day of eligible employment and will end 28 days from the last day of the pay period in which your position ends. At this time, COBRA benefits information will be mailed to you from the COBRA Unit in Albany. To ensure that this information reaches you, you must notify hrs_benefits@stonybrook.edu of any address changes.

SIGNING UP

New Employee

For yourself, spouse, and any dependents you would like to enroll, copies of the Required Proofs must be attached to the NYS Health Insurance Transaction Form (PS-404G)and sent to hrs_benefits@stonybrook.edu within 30 days of your appointment date (deductions will be taken retroactively to cover you from the first day of employment).

LATE ENROLLMENTS

Please note: Late enrollments will be subjected to a 30 day waiting period from the date the enrollment form is received in the Student Health Insurance Office.

Current Employee

Changes to your current health insurance status can be made during the annual open enrollment period in August of each year.

REQUIRED PROOF(S)

No substitutions will be allowed and the Department of Civil Service will not accept any enrollment applications without the required documents.
