REQUEST TO CHANGE LEGAL NAME
If you have had a legal name change, here are the steps you will need to take to ensure our records are updated as well...
State Employees INCLUDING GA/TAS
STEP 1: Bring the following items below to Human Resources. The State Payroll unit will review your request and update SUNY's system of records, Payserv, and our campus system of records, Peoplesoft.
→ Completed State Change Form
→ Copy of the updated Social Security Card with NEW name.
STEP 2: Be sure to also notify other applicable areas as shown here.

RF Employees
INCLUDING RPAS
STEP 1: Bring the following items to Human Resources. The RF Payroll unit will review your request and update RF's system of records, Oracle, and coordinate the updating of our campus system of records, Peoplesoft.
→ Completed RF Change Form
→ Copy of the updated Social Security Card with NEW name.
→ Updated I-9 Form and, if applicable, the Form I-9 Acceptable Documents.
STEP 2: Be sure to also notify other applicable areas as shown here.

BENEFITS
State Employees: Bring a copy of your updated Social Security Card with the NEW name to the Benefits Office. Once they make the change in the NYBEAS system, new benefit cards will be reissued automatically if applicable.
RF Employees: There is no need to notify benefits. Once RF Appointments updates Oracle, your benefit carriers will automatically be notified and new benefit cards will be reissued if applicable.
RETIREMENT PLAN
If you are enrolled in a retirement plan, notify them as well. Retirement Plan Contact Information
UNION
State Employees: If applicable, notify your union (UUP, CSEA, M/C) for an update to any union administered benefit records.
VISA AND IMMIGRATION SERVICES
For non-immigrant employees, please also email vis_scholar@stonybrook.edu.
GOOGLE MAIL
Provided Peoplesoft has been updated with the new name — If you would also like your campus email to reflect this new legal name change, you must
→ Go to https://it.stonybrook.edu/ (Accounts & Passwords -> Request Name or ID Change ->Google E-Mail) to open a ticket with DoIT to request to have your Campus @stonybrook.edu email updated. Questions? Email caccount@stonybrook.edu.
→ If you also have an East Campus @stonybrookmedicine.edu email account, you must also call the UHMC Help Desk @ 631-444-4357 to have that email account updated as well.
CAMPUS CARD
Provided Peoplesoft has been updated with the new name — If you would also like your Campus ID to reflect this new legal name change, you must contact CampusCard@stonybrook.edu.
