Meningitis Requirements

According to New York State Public Health Law 2167, post-secondary institutions must distribute information about meningococcal disease and vaccination to students, or parents/guardians of students under age 18. 

Each student planning to register for at least one in-person class, live on campus, or access in-person services or facilities on Stony Brook’s campuses must satisfy one of the two requirements below:

1. Upload documentation to the Wolfie Health Portal indicating at least one dose of meningococcal ACWY vaccine within the last five years or a complete two or three dose series of Meningitis B.

2. Print, read, sign and upload theMeningitis Response Form to the Wolfie Health Portalindicating that the student will not obtain immunization against meningococcal disease and understands the risks associated with not receiving the vaccination. Students 17 or under must have a parent sign this form.
