Proof of Vaccination 

Thank you again to all of the employees who have reported completion of their COVID-19 vaccine series.

While you have shared your vaccination status with us in SOLAR,  you must also provide proof of full vaccination to the Office of Human Resources on or before November 8, 2021.

How to provide proof?

Fax - Fax a copy of your vaccine card to HR at (631) 632-6621 

In-Person - Bring a copy of your vaccine card to HR
