IMPORTANT COVID-19 MESSAGE – FEDERAL VACCINE MANDATE
The Office of the Vice President for Research has notified all Research Foundation (RF) employees at SBU about Executive Order 14042, “Ensuring Adequate COVID Safety Protocols for Federal Contractors.” This order applies to all SBU RF employees whether they are working full time, part time or remotely (except in limited circumstances where an employee is legally entitled to an accommodation), and requires full COVID-19 vaccination by December 8, 2021. An employee is considered fully vaccinated for COVID-19 two weeks after receiving the second dose in a two-dose vaccine series, or two weeks after receiving a single-dose vaccine, as outlined below.
If you have already submitted proof of full COVID-19 vaccination, thank you for your submission. There is no need to read further and you may disregard this notice.
If you fall into one of the categories below, you will need to submit proof of vaccination as soon as possible, but no later than 5 pm on Wednesday, November 24, 2021:
● You are not yet vaccinated (see ‘Vaccine Opportunities’ below)
● You are fully vaccinated, and have not yet submitted proof of full vaccination to Human Resources
● You are fully vaccinated, have confirmed your vaccination status in SOLAR, but have not ALSO provided proof of your COVID-19 vaccination to Human Resources
PLEASE NOTE: If you do not provide evidence of vaccination by the end of business on November 24, and you do not have an approved or pending medical or religious exemption (see below), you will be required to meet with an Office of Employee and Labor Relations representative to discuss your non-compliance with the order. You will receive a separate notification of the date and time of your interview.
HOW TO PROVIDE PROOF:
● Fax – Fax a copy of your vaccine card to HR at (631) 632-6621

● In-Person – Bring a copy of your vaccine card to HR, Administration Building, Room 390, Monday - Friday, 9 am - 4 pm
HOW TO MEET THE DECEMBER 8 DEADLINE IF YOU’RE NOT VACCINATED YET:
According to the CDC, in order to meet the Executive Order’s December 8 deadline for compliance, the latest possible dates to begin the vaccination process for each of the three available vaccines are:
Oct. 27, 2021 for first dose of Moderna (2 doses needed, 4 weeks apart + 2 weeks to take full effect)
Nov. 3, 2021 for first dose of Pfizer (2 doses needed, 3 weeks apart + 2 weeks to take full effect)
Nov. 24, 2021 for Johnson & Johnson (1 dose needed + 2 weeks to take full effect.
VACCINE OPPORTUNITIES:
Appointments are available at our vaccination site located in the Innovation & Discovery Building (IDC) in Stony Brook’s Research and Development (R&D) Park. Currently, the site is open 8:00 am - 6:15 pm, seven days a week, with a daily limited number of walk-in appointments available. Click here for more SBU vaccine information.
You may also visit any State-run COVID-19 vaccine site. To check availability throughout the area (including the University’s site), visit the New York State “Am I Eligible” website. Walk-ins are welcome, but you can also use the NYS site to schedule an appointment.
EXEMPTIONS:
Only exemption requests for medical and religious reasons will be considered and must be submitted to the Health Information Line for medical (access medical exemption form here) and the Office of Equity and Access for religious exemption requests (access religious exemption form here) no later than Monday, November 8, 2021. Those employees whose exemption requests are pending determination as of December 8 will be placed on leave without pay until a determination on their request is made.
