IMPORTANT MESSAGE REGARDING NEW DEADLINE FOR THE FEDERAL COVID-19 VACCINE MANDATE

IMPORTANT MESSAGE REGARDING NEW DEADLINE FOR THE FEDERAL COVID-19 VACCINE MANDATE

The White House announced that the date by which Federal Contractors and employees of Federal Contractors must be fully vaccinated for COVID-19 has been extended to January 18, 2022. Therefore, all Research Foundation employees must provide proof of a second dose of a two dose vaccine series, or a single dose of the Johnson and Johnson vaccine by 5pm on January 4, 2022 in order to meet the revised January 18, 2022 mandate. An employee is considered fully vaccinated for COVID-19 two weeks after receiving the second dose in a two dose vaccine series, or two weeks after receiving the single dose vaccine (see ‘How to Meet the Deadline’ below).  

Executive Order 14042, “Ensuring Adequate COVID Safety Protocols for Federal Contractors,” applies to all Research Foundation employees at Stony Brook University, whether they are full-time, part-time or working remotely (except in limited circumstances where an employee is legally entitled to a medical or religious accommodation), and requires full COVID-19 vaccination by January 18, 2022. 

WHAT ACTION SHOULD YOU TAKE?

If you have already submitted proof of full COVID-19 vaccination or you are covered under the DOH mandate and have already provided proof of full vaccination, thank you for your submission. There is no need to read further and you may disregard this notice.

If you fall into one of the categories below, you will need to submit proof of vaccination as soon as possible, but no later than 5 pm on January 4, 2022:

  • You are not yet vaccinated (see ‘Vaccine Opportunities’ below)

  • You are fully vaccinated, and have not yet submitted proof of full vaccination to Human Resources.

  • You are fully vaccinated, have confirmed your vaccination status in SOLAR, but have not ALSO provided proof of your COVID-19 vaccination to Human Resources.

PLEASE NOTE: If you do not provide proof of a second dose of a two dose vaccine series, or a single dose of the Johnson and Johnson vaccine by 5pm on January 4, and you do not have an approved or pending medical or religious accommodation (see below), you will be required to meet with an Office of Employee and Labor Relations representative to discuss your non-compliance with the order. You will receive a separate notification of the date and time of your interview.

HOW TO PROVIDE PROOF:

  • Fax – Fax a copy of your vaccine card to HR at (631) 632-6621

    OR

  • In-Person – Bring a copy of your vaccine card to HR, Administration Building, Room 390, Monday - Friday, 9 am - 4 pm

HOW TO MEET THE DEADLINE:

According to the CDC, in order to meet the Executive Order’s January 18, 2022 deadline for compliance, the latest possible dates to begin the vaccination process for each of the three available vaccines are:

  • No later than Dec. 7, 2021 for first dose of Moderna, and no later than January 4, 2022 for the second dose of Moderna (2 doses needed, 28 days apart + 2 weeks to take full effect)

  • No later than Dec. 14, 2021 for first dose of Pfizer, and no later than January 4, 2022 for the second dose of Pfizer (2 doses needed, 21 days apart + 2 weeks to take full effect)

  • No later than January 4, 2022 for Johnson & Johnson (1 dose needed + 2 weeks to take full effect.

VACCINE OPPORTUNITIES:

Appointments are available at our SBU vaccination site located in the Innovation & Discovery Building (IDC) in Stony Brook’s Research and Development (R&D) Park. Currently, the site is open Monday through Saturday, 8:00 am to 6:30 pm; Sunday, 8 am to noon, with a daily limited number of walk-in appointments available. To make an appointment here or at any other State-run COVID-19 vaccine site visit am-i-eligible.covid19vaccine.health.ny.gov.

EXEMPTIONS:

Only exemption requests for medical and religious reasons will be considered and must be submitted to the Health Information Line for medical (access medical exemption form here) and the Office of Equity and Access for religious exemption requests (access religious exemption form hereno later than December 13, 2021. Those employees whose exemption requests are pending determination on or after January 18, 2022 will be placed on leave without pay until a determination on their request is made.
