Responsibilities & Requirements
The Nurse Practitioner will see and treat patients with diabetes admitted to Stony Brook University Hospital and will demonstrate skills in collaboration, teamwork and problem solving to achieve desired goals.
- Independently assess and evaluate new and follow-up consults to the inpatient diabetes consultation service at Stony Brook University Hospital.
- Perform daily physical exams of patients with diabetes with special attention to body systems that may be affected by diabetes.
- Review 24 hour tests of blood glucose values and recommend daily orders for medication to primary clinical team.
- Educate patients and family members regarding diabetes self-management principles based on recognized curricula and national standards of care.
- Identify discharge needs at the point of initial patient contact and assist in the transition of care planning.
- Rotate through the established call schedule of the diabetes service and carry a paging device.
- Other duties as assigned.
