Accreditation-Ready New CORIHS Applications Now Live in IRBNet

ACCREDITATION-READY NEW CORIHS APPLICATIONS ARE NOW LIVE IN IRBNET

Investigators creating IRBNet packages that are new or continuing review CORIHS submissions will now be able to make use of our new review forms, developed in preparation for our upcoming application to our accreditation organization, AAHRPP.

Summary:

  • For new, first time submissions, you will need to complete an ‘on-line’ Registration Form, an Application for Expedited or Full CORIHS Review, and all other materials required for such submissions, as outlined in CORIHS Submission Requirements for New, First Time Submissions. This may also include new Supplemental forms if your project is proposing use of questionably viable or nonviable neonates, international sites, or, prisoners.
  • For regular (years 1-4) continuing review submissions, you will need to complete an ‘on-line’ Registration Form, an Application for Continuing Expedited or Full CORIHS Review (Years 1-4) , and all other materials required for such submissions, as outlined in CORIHS Submission Requirements for Continuing Reviews (Regular and Five-Year)
  • For five (5) year continuing review submissions, you will need to complete an ‘on-line’ Registration Form, an Application for Five Year Continuing Expedited or Full CORIHS Review, and all other materials required for such submissions, as outlined in CORIHS Submission Requirements for Continuing Reviews (Regular and Five-Year). This may also include new Supplemental forms if your project is proposing use of questionably viable, or nonviable neonates, international sites, or, prisoners.

Details:

  • Use of the Registration Form for Expedited or Full CORIHS Review: This is a short ‘smart’ or ‘wizard’ form that quickly captures critical, searchable data pertaining to your study. To access this document:
    • Go to your project’s Designer
    • Select ‘Add New Document’
    • In the lower shaded ‘On-Line Document’ box, select ‘Registration Form for Expedited or  Full Committee Review’, then hit the ‘Add’ button
    • Begin completing the Registration Form. You do not need to complete the whole form in one sitting; you can ‘save and exit’, and then go back and update the document by clicking the pencil icon next to the document listed in the designer.
    • When you are done completing the registration form, click “Preview” to see what the completed form looks like, and confirm that all the information provided is accurate. Once you are satisfied in this regard,

Click "Save and Exit", and then continue constructing your project package to completion:

  • If you are submitting a new project in IRBNet:
    • Complete the Registration Form as detailed above
    • Download and complete the "Application for Expedited or Full CORIHS Review", a downloadable document located in the project’s Designer Library that will capture more detail about the answers you provided in the Registration Form.  This second form will evolve over time, to facilitate its use, and to ensure compliance with our upcoming accreditation requirements.
    • Continue adding documents (click ‘Add a Document’ in the Designer) for your submission, in accordance with the requirements outlined in the CORIHS Submission Requirements for New, First Time Submissions, also available in the project’s Designer library. This may also include Supplemental forms if your project is proposing use of questionably viable or nonviable neonates, international sites, or, prisoners.
    • Obtain appropriate signatures and submit the package to the CORIHS office, in accordance with the Instructions for SBU or BNL Investigators.

 

  • If you are submitting a ‘Regular’ (years 1-4) Continuing Review package into IRBNet and you have not previously submitted the electronic Registration Form for your project:
    • Complete the Registration Form as detailed above
    • Complete the "Application for Continuing Expedited or Full CORIHS Review (Years 1-4)” located in the project’s Designer Library within IRBNet.
    • Continue adding documents (click ‘Add a Document’ in the Designer) for your submission, in accordance with the requirements outlined in the CORIHS Submission Requirements for Continuing Reviews (Regular and Five-Year) , also available in your project’s Designer library.
    • Obtain appropriate signatures and submit the package to the CORIHS office, in accordance with the Instructions for SBU or BNL Investigators

 

  • If you are submitting a Five Year Continuing Review package into IRBNet:
    • Complete the Registration Form as detailed above
    • Complete the "Application for Five Year Continuing Expedited or Full CORIHS Review", located in the project’s Designer Library within IRBNet.
    • Continue adding documents  (click ‘Add a Document’ in the Designer) for your submission, in accordance with the requirements outlined in the CORIHS Submission Requirements for Continuing Reviews (Regular and Five-Year) also available in your project’s Designer library. This may also include new Supplemental forms if your project is proposing use of questionably viable or nonviable neonates, international sites, or, prisoners.
    • Obtain appropriate signatures and submit the package to the CORIHS office, in accordance with the Instructions for SBU or BNL Investigators.

Submissions to the IRBNet CORIHS office that do not include the registration form, the appropriate CORIHS application, and all other required materials as discussed above will be considered incomplete and will not be forwarded to the committees for action. if you have any questions, please contact the Office of Research Compliance at 631-632-9036.
